Locality:United States. Kentucky. Madison: Taylor Fork Ecological Area. Take Kit Carson Drive S, and then head E onto McKinney Skills Drive. Turn S again towards the observatory, and follow that road until you reach the second gate. Inside that gate head to the W over the bridge and follow the trail up the hill. Once you reach the top the specimen will be on the W before you reach the trail sign on Mammals.; 37.716092 -84.294737 37° 42' 57" N, 84° 17' 41" W
Description:Smal white flower with narrow dark streaking on the petals. Small diameter stem with narrow basal leaves. Roots are white and unbranched. 
Habitat:Lowland area, with clay soil and a moderate amount of shade.
